        $750.00 additional for the first month if you proceed with prescription treatment and counseling

        $325 per month for months 2 and 3

        $225 per month for months 4 and 5

        $150 per month each additional month

            $350.00 additional for the first month if you proceed with ongoing treatment and counseling

        $250 consult

        $350.00 additional for the first month if you proceed with prescription treatment and counseling.
